Applying for Leeds City Council transport?

This application form is for:

  • Children and young people with SEND or a medical condition, aged 5 to 19, who attend a school or a SILC, 

      Or

  • Parents of primary-aged children who have a disability which prevents them from accompanying their child on the walking route between home and their nearest qualifying school. 

 

Instructions

It should take you about 20 to 30 minutes to complete this form. You can save your progress at any point and return to the form later.

Please note that once your fully-completed application has been submitted, it may take the council up to 15 working days to assess your application and to respond to you in writing.

 

Supporting your child's transport journey

To help your child feel more confident about their journey to school, you can find a transport social story at the bottom of this page. This story can be shared with your child to help them understand what to expect when using the school bus or taxi. 

If you would like a paper copy, please contact the school office, and we can arrange to send one home.

You’ll also find some useful telephone numbers on the last page of the story. These are numbers you can call if the transport is running late or if you need help once your child has started school and transport has been agreed.
